Patients with hypertension can now use this new stress free algorithm for treatment

Hypertensive patients can now receive intensive treatments faster. In a recent study, researchers have devised a machine learning algorithm which combines three variables routinely collected during clinic visits and demonstrates how the emerging field of bioinformatics could transform patient care.

It takes a patient’s age, urinary albumin/creatinine ratio (UACR), and cardiovascular disease history to successfully identify hypertensive patients for whom the benefits of intensive therapy outweigh the risks. “Large randomized trials have provided inconsistent evidence regarding the benefit of intensive blood pressure lowering in hypertensive patients,” said a researcher, Yang Xie.

“To the best of our knowledge, this is the first study to identify a subgroup of patients who derive a higher net benefit from intensive blood pressure treatment,” he added. Researchers used patient data under controlled trials that tested intensive vs. standard blood pressure-lowering treatments — the Systolic Blood Pressure Intervention Trial (SPRINT) and the Action to Control Cardiovascular Risk in Diabetes (ACCORD) trial.

The SPRINT trial included 9,361 non-diabetic hypertensive adults at an elevated risk of a cardiovascular event, while ACCORD enrolled 10,251 patients with Type 2 diabetes. “I think our algorithm can help us identify high-risk patients who will most likely benefit from intensive blood pressure reduction. Long-term intensive HBP drug therapy can reduce the risk of heart failure and death, but it carries an increased risk of side effects,” said another researcher, Wanpen Vongpatanasin.

The researchers’ machine learning method determined three simple criteria to identify adults with high blood pressure who are at the highest risk for early major adverse cardiovascular events — such as cardiovascular death, heart attack, or stroke.

Severe headache with fever? It could be a symptom of viral infection

Running a high fever and having headaches are two different symptoms that can be shrugged off when seen separately. However, severe headache with fever may just be a sign of something more serious. While such an eventuality need not necessarily mean a life-threatening illness, it is imperative that you visit a specialist at the earliest for a proper diagnosis and course of treatment.

“Headaches may be a cause for concern when accompanied by a fever, besides being a sign of some serious infection. This infection could be localised to the brain and/or spinal cord. For instance, meningitis, encephalitis or a brain abscess,” says Dr Haresh Tolia, a Delhi-based general physician who consults for medical app Lybrate.

“Apart from this, flu or other systemic or whole-body infections can also lead to headache and fever. Other possible health conditions are bleeding or a tumour in the brain. In most cases, these could be symptoms of some run-of-the-mill viral infection that needs to run its course before subsiding,” adds Dr Tolia.

So, what should one do when experiencing a fever headache? “Do not ignore any headache with high or low-grade fever. Consult a doctor to rule out any underlying health issues. It can be simple dengue fever, chikungunya, malaria, acute sinusitis or flu, or serious brain fevers like meningitis or encephalitis. The red flags are altered mental status, vomiting and neck stiffness,” explains Dr K K Aggarwal, national president of the Indian Medical Association.

Dr Aggarwal suggests certain precautions (and home remedies) for those suffering from a fever headache.

1. Paracetamol can provide relief from pain and aches, besides fever.

2. Wet a piece of cloth and dab it at regular intervals on the forehead, arms and legs. This can help bring down fever.

3. Do not overheat yourself. Remove any extra piece of clothing. Overheating can cause the body temperature to rise and make symptoms worse.

4. Eat healthy foods. You can even opt for soups and other liquid foods to make you feel better.

Post heart attack, cardiac muscles could recover with new treatment

Scientists have identified a new way to repair the damaged heart muscles and also recover almost all of its pumping function after a heart attack.

During a heart attack, the organ is starved of oxygen and the muscles are damaged. The body responds by sending in immune cells to clear up dead and dying cells, but these cells themselves cause further inflammation in the already damaged heart further leading to heart failure.

For the study, published in Journal of Clinical Investigation, the team injected a protein called VEGF-C in mice after heart attack.

They found that the VEGF-C treatment promoted growth of a network of vessels which are part of the lymphatic system. The extra lymphatic vessels allow the immune cells to be quickly cleared after helping to repair and clear dying and dead cells, before they can cause significant damage from inflammation.

This treatment led to better healing in the heart and better recovery of the heart’s pumping function after a heart attack, according to the team.

Further, after clearing away macrophages, the first line of immune cells that start the healing process, other immune cells called Tregs could then be allowed into the injured muscle, to further help the heart to repair and recover.

“We started looking at the lymphatic system in the heart a few years ago – we could never have known how pivotal it might turn out to be for heart repair,” said Paul Riley from University of Oxford.

“This research has allowed us to start a drug discovery programme to develop drugs to promote the growth of lymphatic vessels and quickly clear immune cells. We hope to get a treatment we could give to people after a heart attack within five to 10 years,” Riley added.

Deaths due to cardiovascular disease up in rural areas of India: Study

While cardiovascular disease is the cause of over a quarter of Indian deaths each year, in a counter-intuitive trend, the mortality rates for rural populations due to this condition have surpassed those in urban areas, according to a new study, published on Friday.

The study is titled “Divergent trends in ischaemic heart disease and stroke mortality in India from 2000 to 2015: a nationally representative mortality study”. Cardiovascular disease caused 2.1 million deaths in India in 2015, over a fourth of the total. For those aged between 30 and 69 years, nearly 70% of the deaths were due to ischaemic heart disease — caused by narrowing of the arteries, a condition that often culminates in fatal heart attacks.

For that age bracket, the probability of dying from that cause increased from 10.4% to 13.1% for men and from 4.8% to 6.6% for women. “Although ischaemic heart disease mortality at ages 30–69 years was lower in rural areas compared with urban areas at the start of the study, rural rates rose rapidly, surpassing urban rates by 2015 in both sexes,” the study found.

“The assumption is that urbanisation is leading to rising rates of heart disease. But in fact, what we showed is that it’s the rural areas of India that had an increase in ischaemic heart disease mortality. In the urban areas, it’s been flat or there’s been a modest decline. That suggests a significant role for untreated hypertension or undertreated diabetes,” Prabhat Jha, director of the Centre for Global Health Research at St Michael’s Hospital in Toronto and senior author of the study said.

This research is based on data from the Million Death Study, a large-scale exercise that has been in progress in India since 2001 and was conducted in collaboration with the Registrar General of India and medical agencies in the country.

Another significant finding was that younger adults, especially those born after 1970, have the highest rate of death due to heart problems caused by narrowing of the heart’s arteries.

“As India has gotten fatter, including in the urban and rural areas, hypertension and diabetes have increased substantially, but the treatment for those have lagged behind. That might be what’s at play here,” Jha said.

The study also examined prevalence of death due to stroke and found “the probability of dying from stroke decreased overall, but increased in India’s North-Eastern states, where a third of premature stroke deaths occurred and only one sixth of (India’s) population lives.

Children with other genetic disorders like fragile X syndrome or who have siblings with autism are also at increased risk. However, environmental or even infectious factors are also being investigated as potential contributors, as well.

Until we have a better understanding of what is causing autism, it will be hard to determine how to develop effective treatments. There is currently no cure for autism, but interventions early in a child’s life – especially between birth and three years old – can significantly improve children’s social skills and development. Among other things, behavioral therapy and specialized educational programs can focus on helping children with autism talk and interact with others. Consequently, it’s important to identify potential warning signs as early as possible.

People with autism generally have trouble with social, emotional and communication skills that persist throughout their lifetimes. The symptoms of autism vary in severity along a spectrum, so mild cases may go undiagnosed for many years. Some signs and symptoms may begin during infancy. According to the National Institute of Health, early indicators can include no babbling or pointing by age 1, no single words by 16 months or two-word phrases by age 2, not responding to their name, losing language or social skills they previously had, poor eye contact, performing repetitive tasks excessively, or a lack of smiling or being socially responsive. Older children may have trouble making friends and having sustained conversations. They might also engage in less imaginative playing, have repetitive or unusual speaking habits, or have interests or preoccupations that are very specific and restricted. Diabetes is a chronic disease that affects millions of people worldwide. Uncontrolled cases can cause blindness, kidney failure, heart disease and other serious conditions. Before diabetes is diagnosed, there is a period where blood sugar levels are high but not high enough to be diagnosed as diabetes. This is known as prediabetes.

Coffee and tea have antioxidants known as polyphenols that may help protect against diabetes  In addition, green tea contains a unique antioxidant compound called epigallocatechin gallate (EGCG) that has been shown to reduce blood sugar release from the liver and increase insulin sensitivity.

An ear infection can occur in any of the three parts. Inner ear infections are the most serious. It is important to treat an ear infection to prevent complications, including damaging or rupturing the ear drum. Untreated ear infections can also lead to chronic recurrence and can even cause deafness! It is important to consult a doctor, but for immediate relief you can try some natural treatments.There are many time-honored, simple and easy home remedies that can help to get rid of the pain and other symptoms of an ear infection.

1. Salt

Heat up one cup of salt on a pan over low heat for a few minutes. You can also heat it in a microwave or double-boiler.Place the hot salt on a cloth and seal the open end with rubber band (or tie a knot).When it is bearably hot, lay down and put the cloth on the affected ear for 5 to 10 minutes. Repeat this remedy daily as many times as needed. The heat generated from the sock will help draw out fluid from the ear and relieve swelling and pain.

2. Holy Basil or Tulasi

Crush four to five fresh holy basil leaves gently to extract the juice. Apply the basil juice on or around the infected ear. Avoid getting the juice in the ear canal.You can also mix a few drops of holy basil oil with an equal amount of carrier oil like coconut oil. Soak a cotton ball in the mixture and gently wipe just inside the ear, around the outer edge and behind the ear. Repeat the process twice daily.

3. Olive Oil 

One of the main causes of an ear infection is wax in the ear catching some fungal or bacterial growth leading to a blockage in the Eustachian tubes. You can easily clear the obstruction with the help of olive oil.

Warm some olive oil slightly. Put a few drops of the warm oil into the infected ear. The oil will cause the wax to soften. Remove the infected wax with cotton-tipped swabs. Be careful not to put the swab too far in the ear or you might damage the eardrum.

4.  Onion Juice

Onion is a very common ingredient used in cooking. It has medicinal uses too, including for the treatment of an ear infection.

Chop one small onion, put it in a bowl and microwave it for one to two minutes. Allow it to cool and then strain out the onion juice. Put two to three drops of the juice in the infected ear, leave it for sometime and then turn your head to let it drain out of your ear. Place the cloth on the infected ear for five minutes. Wait for 10 minutes and then repeat the process.

5.  Mango leaf extract
