WHAT IS OTITIS MEDIA?

Otitis media refers to inflammation of the middle ear. When infection occurs, the condition is called “acute otitis media.” Acute otitis media occurs when a cold, allergy, or upper respiratory infection, and the presence of bacteria or viruses lead to the accumulation of pus and mucus behind the eardrum, blocking the Eustachian tube. This causes earache and swelling.

When fluid forms in the middle ear, the condition is known as “otitis media with effusion.” This occurs in a recovering ear infection or when one is about to occur. Fluid can remain in the ear for weeks to many months. When a discharge from the ear persists or repeatedly returns, this is sometimes called chronic middle ear infection. Fluid can remain in the ear up to three weeks following the infection. If not treated, chronic ear infections have potentially serious consequences such as temporary or permanent hearing loss.

HOW DOES OTITIS MEDIA AFFECT A CHILD’s HEARING?

All children with middle ear infection or fluid have some degree of hearing loss. The average hearing loss in ears with fluid is 24 decibels…equivalent to wearing ear plugs. (Twenty-four decibels is about the level of the very softest of whispers.) Thicker fluid can cause much more loss, up to 45 decibels (the range of conversational speech).

Your child may have hearing loss if he or she is unable to understand certain words and speaks louder than normal. Essentially, a child experiencing hearing loss from middle ear infections will hear muffled sounds and misunderstand speech rather than incur a complete hearing loss. Even so, the consequences can be significant – the young patient could permanently lose the ability to consistently understand speech in a noisy environment (such as a classroom) leading to a delay in learning important speech and language skills.

TYPES OF HEARING LOSS

Conductive hearing loss is a form of hearing impairment due to a lesion in the external auditory canal or middle ear. This form of hearing loss is usually temporary and found in those ages 40 or younger. Untreated chronic ear infections can lead to conductive hearing loss; draining the infected middle ear drum will usually return hearing to normal.

The other form of hearing loss is sensorineural hearing loss, hearing loss due to a lesion of the auditory division of the 8th cranial nerve or the inner ear. Historically, this condition is most prevalent in middle age and older patients; however, extended exposure to loud music can lead to sensorineural hearing loss in adolescents.

WHEN SHOULD A HEARING TEST BE PERFORMED?

A hearing test should be performed for children who have frequent ear infections, hearing loss that lasts more than six weeks, or fluid in the middle ear for more than three months. There are a wide range of medical devices now available to test a child’s hearing, Eustachian tube function, and reliability of the ear drum. They include the otoscopy, tympanometer, and audiometer.

Do children lose their hearing for reasons other than chronic otitis media?

Children can incur temporary hearing loss for other reasons than chronic middle ear infection and Eustachian tube dysfunction. They include:

  • Cerumen impaction (compressed earwax)
  • Otitis externa: Inflammation of the external auditory canal, also called “swimmer’s ear.”
  • Cholesteatoma: A mass of horn shaped squamous cell epithelium and cholesterol in the middle ear, usually resulting from chronic otitis media.
  • Otosclerosis: This is a disease of the otic capsule (bony labyrinth) in the ear, which is more prevalent in adults and characterized by formation of soft, vascular bone leading to progressive conductive hearing loss. It occurs due to fixation of the stapes (bones in the ear). Sensorineural hearing loss may result because of involvement of the cochlear duct.
  • Trauma: A trauma to the ear or head may cause temporary or permanent hearing loss.

Ear health Tips:

  1. Use earplugs around loud noises.
  2. Turn the volume down when you are listening to music using head phone.
  3. Give your ears time to recover after exposing to loud noice.
  4. Stop using cotton swabs in your ears, don’t clean ears. They are self cleaning organs. Wax will come out. If you see, clean with damp towel.

How to identify psoriasis and ringworm

Both psoriasis and ringworm cause red, scaly patches on the skin. Knowing the symptoms of each condition can help people to identify their rash.

Psoriasis and ringworm are very different conditions. Ringworm is a fungal infection that will go away with treatment. Psoriasis is a chronic autoimmune condition that comes and goes throughout a person’s life.

When someone develops a rash, they may find it difficult to work out what caused it. A doctor can examine or order tests to determine which condition they have.

In this article, we look at the differences between ringworm and psoriasis and compare them with other conditions that can cause similar rashes.

Both psoriasis and ringworm can cause red, scaly patches to appear anywhere on the body. Both can also lead to intense itching.

A person who has never had ringworm before or is having their first psoriasis flare may not know which condition they have.

Psoriasis is an autoimmune condition. It causes red, scaly plaques that may have a gray hue or peel. During a psoriasis flare, a person develops scaly, red, irritated patches of skin because of the skin’s rapid shedding.

Psoriasis is not contagious. Researchers are not sure what causes psoriasis, but it tends to run in families. Certain factors, such as stress or alcohol, can trigger a flare.

Ringworm causes a red, circular, and often scaly rash. Ringworm is highly contagious, spreading quickly through contact with another person who has the condition.

A fungal infection causes ringworm. Despite its name, it has nothing to do with worms.

Both psoriasis and ringworm can develop in similar places. One type of psoriasis called inverse psoriasis can affect the area around the groin. When ringworm affects the groin area, it is called jock itch.

Psoriasis often affects the scalp. When ringworm affects the scalp, it is called tinea capitis.

Some important distinctions between psoriasis and ringworm include:

The shape of the rash

Ringworm is often circular. It typically begins as a rash with a clearly defined border that looks like there is a worm under the skin. The center of the rash appears sunken and may be gray or scaly.

While psoriasis rashes can also be round, the shape is less regular and does not resemble a worm.

Rash risk factors

People develop ringworm after coming into contact with someone else who has the infection. Children, anyone in close contact with children, and people who come into contact with others, such as at the gym, are more likely to get the infection.

Psoriasis is not contagious and can affect anyone.

Rash growth

Psoriasis plaques may spread, appearing on one area of the body and then another.

Ringworm grows larger over several days and can spread to other areas of the body.

Changes in the rash

Unlike ringworm, psoriasis plaques can change in color and texture. They are often red at first and then may become grey and scaly, or crack and bleed.

How the rash feels

Both ringworm and psoriasis rashes are itchy, but the ringworm itch is often more intense. Psoriasis can be painful and may tingle or burn. Some people develop other symptoms with psoriasis, such as a fever or muscle aches.

Heat rash

Sweating or being exposed to high temperatures can lead to a heat rash. For example, a person might develop a heat rash on their legs after sitting in front of a space heater.

The rash is not usually painful and goes away on its own in a few days. In some cases, a person might need to use corticosteroid cream to speed healing.

Allergic reaction

An allergic reaction can trigger an outbreak of hives. Allergic reactions typically appear suddenly, either because of something that came into contact with the skin or a food allergen.

Allergic reactions can cause other symptoms, too, such as difficulty breathing or feeling sick.

Dry skin

Although psoriasis plaques can look very dry and scaly, dry skin does not cause them. Dry skin can crack open and bleed, and may peel. Unlike psoriasis, dry skin does not change color or shape.

Skin blisters

Some types of psoriasis, including guttate psoriasis, look like tiny blisters. A blister due to skin irritation, such as from a shoe rubbing the foot, typically develops over several days, then bursts and goes away. If the blisters do not go away or are spreading, it could be psoriasis or another skin disorder.

Treating psoriasis and ringworm

Doctors can treat ringworm using antifungal creams or oral antifungal medication. With the correct treatment, ringworm usually goes away within 2 to 4 weeks.

People with low immunity or debilitating conditions may develop severe infections due to ringworm. To prevent the infection from getting worse, they may need more aggressive treatment.

Psoriasis is a chronic condition. Flares come and go over time. There is no cure, though people can manage their symptoms using creams, medications, or light therapy. They can also reduce the risk of flares by avoiding triggers, such as stress or drinking alcohol.

What is Indian Gooseberry (Amla)?

Indian gooseberry or amla (Emblica officinalis or Phyllanthus emblica) is an extremely sour, nutritious fruit of a tree which grows in India, the Middle East, and a few other Southeast Asian countries. Ovarian Cancer

Signs and Symptoms of Ovarian Cancer

Ovarian cancer may cause several signs and symptoms. Women are more likely to have symptoms if the disease has spread, but even early-stage ovarian cancer can cause them. The most common symptoms include:

  • Bloating
  • Pelvic or abdominal (belly) pain
  • Trouble eating or feeling full quickly
  • Urinary symptoms such as urgency (always feeling like you have to go) or frequency (having to go often)

These symptoms are also commonly caused by benign (non-cancerous) diseases and by cancers of other organs. When they are caused by ovarian cancer, they tend to be persistentand a change from normal − for example, they occur more often or are more severe. These symptoms are more likely to be caused by other conditions, and most of them occur just about as often in women who don’t have ovarian cancer. But if you have these symptoms more than 12 times a month, see your doctor so the problem can be found and treated if necessary.

Others symptoms of ovarian cancer can include:

  • Fatigue (extreme tiredness)
  • Upset stomach
  • Back pain
  • Pain during sex
  • Constipation
  • Changes in a woman’s period, such as heavier bleeding than normal or irregular bleeding
  • Abdominal (belly) swelling with weight loss

Ovarian Cancer Risk Factors

A risk factor is anything that changes your chance of getting a disease like cancer. Different cancers have different risk factors. Some risk factors, like smoking, can be changed. Others, like a person’s age or family history, can’t be changed.

But having a risk factor, or even many, does not mean that you will get the disease. And some people who get the disease may not have any known risk factors. Researchers have discovered several risk factors that might increase a woman’s chance of developing epithelialovarian cancer. These risk factors don’t apply to other less common types of ovarian cancer like germ cell tumors and stromal tumors.

Factors that increase your risk of ovarian cancers

Getting older

The risk of developing ovarian cancer gets higher with age. Ovarian cancer is rare in women younger than 40. Most ovarian cancers develop after menopause. Half of all ovarian cancers are found in women 63 years of age or older.

Being overweight or obese

Obesity has been linked to a higher risk of developing many cancers. The current information available for ovarian cancer risk and obesity is not clear. Obese women (those with a body mass index [BMI] of at least 30) may have a higher risk of developing ovarian cancer, but not necessarily the most aggressive types, such as high grade serous cancers. Obesity may also affect the overall survival of a woman with ovarian cancer.

Having children later or never having a full-term pregnancy

Women who have their first full-term pregnancy after age 35 or who never carried a pregnancy to term have a higher risk of ovarian cancer.

Using fertility treatment

Fertility treatment with in vitro fertilization (IVF) seems to increase the risk of the type of ovarian tumors known as “borderline” or “low malignant potential” (described in What Is Ovarian Cancer?). Other studies, however, have not shown an increased risk of invasive ovarian cancer with fertility drugs. If you are taking fertility drugs, you should discuss the potential risks with your doctor.

Taking hormone therapy after menopause

Women using estrogens after menopause have an increased risk of developing ovarian cancer. The risk seems to be higher in women taking estrogen alone (without progesterone) for many years (at least 5 or 10). The increased risk is less certain for women taking both estrogen and progesterone.

Having a family history of ovarian cancer, breast cancer, or colorectal cancer

Ovarian cancer can run in families. Yourovarian cancer risk is increased if your mother, sister, or daughter has (or has had) ovarian cancer. The risk also gets higher the more relatives you have with ovarian cancer. Increased risk for ovarian cancer can also come from your father’s side.

What Causes Ovarian Cancer?

We don’t yet know exactly what causes most ovarian cancers. As discussed in Ovarian Cancer Risk Factors, we do know some factors that make a woman more likely to develop epithelial ovarian cancer. Much less is known about risk factors for germ cell and stromal tumors of the ovaries.

The most recent and important finding about the cause of ovarian cancer is that it starts in cells at the tail ends of the fallopian tubes and not necessarily in the ovary itself. This new information may open more research studies looking at preventing and screening for this type of cancer. 

There are many theories about the causes of ovarian cancer. Some of them came from looking at the things that change the risk of ovarian cancer. For example, pregnancy and taking birth control pills both lower the risk of ovarian cancer. Since both of these things reduce the number of times the ovary releases an egg (ovulation), some researchers think that there may be some relationship between ovulation and the risk of developing ovarian cancer.

Can Ovarian Cancer Be Prevented?

Most women have one or more risk factors for ovarian cancer. But most of the common factors only slightly increase your risk, so they only partly explain the frequency of the disease. So far, what is known about risk factors has not translated into practical ways to prevent most cases of ovarian cancer.

There are several ways you can reduce your risk of developing the most common type of ovarian cancer, epithelial ovarian cancer. Much less is known about ways to lower the risk of developing germ cell and stromal tumors of the ovaries, so this information does not apply to those types. It is important to realize that some of these strategies lower your risk only slightly, while others lower it much more. Some strategies are easily followed, and others require surgery. If you are concerned about your risk of ovarian cancer, talk to your health care professionals. They can help you consider these ideas as they apply to your own situation.

Avoiding certain risk factors

Some risk factors for ovarian cancer, like getting older or having a family history, cannot be changed. But women might be able to lower their risk slightly by avoiding other risk factors, for example, by staying at a healthy weight, or not taking hormone replacement therapy after menopause. Oral contraceptives

Using oral contraceptives (birth control pills) decreases the risk of developing ovarian cancer for average risk women and BRCA mutation carriers , especially among women who use them for several years. Women who used oral contraceptives for 5 or more years have about a 50% lower risk of developing ovarian cancer compared with women who never used oral contraceptives. Still, birth control pills do have some serious risks and side effects such as slightly increasing breast cancer risk. Women considering taking these drugs for any reason should first discuss the possible risks and benefits with their doctor.

Gynecologic surgery

Both tubal ligation and hysterectomy may reduce the chance of developing certain types of ovarian cancer, but experts agree that these operations should only be done for valid medical reasons — not for their effect on ovarian cancer risk.

If you are going to have a hysterectomy for a valid medical reason and you have a strong family history of ovarian or breast cancer, you may want to consider having both ovaries and fallopian tubes removed (called a bilateral salpingo-oophorectomy) as part of that procedure.

Even if you don’t have an increased risk of ovarian cancer, some doctors recommend that the ovaries be removed with the uterus if a woman has already gone through menopause or is close to menopause. If you are older than 40 and you are going to have a hysterectomy, you should discuss the potential risks and benefits of having your ovaries removed with your doctor.

Another option for average risk women who do not wish to have their ovaries removed because they don’t want to lose ovarian function (and go through menopause early) is to have just the fallopian tubes removed (a bilateral salpingectomy) along with the uterus (a hysterectomy). They may choose to have their ovaries removed later. This has not been studied as well as removing both the ovaries and fallopian tubes at the same time, but there is enough information that it may be considered an option to reduce ovarian cancer risk in average risk women.

If you’ve been diagnosed with ovarian cancer, your cancer care team will discuss your treatment options with you. It’s important that you think carefully about each of your choices. Weigh the benefits of each treatment option against the possible risks and side effects.

Signs and Symptoms of Lung Infection in COPD

People with chronic obstructive pulmonary disease (COPD) have inflamed and narrowed airways and damaged air sacs, which makes them more prone to developing lung infections, particularly pneumonia.

How Lung Infection Occurs

Pneumonia happens when bacteria, viruses, and less commonly fungi collect in a person’s air sacs and begin to grow. The air sacs may become filled with pus and fluid, which can make breathing more difficult, cause chest pain, and lead to a cough that is different from a person’s usual chronic cough associated with their disease.

The good news is that pneumonia may be prevented through proper hand washing and vaccinations.

If you or a loved one has COPD, a lung infection very well may occur at some point. While it’s important to focus on preventing infection, you also want to be on top of one if signs and symptoms start to arise. In addition, it can be tricky sometimes to distinguish between a lung infection and a COPD flare.

Symptoms of a Lung Infection 

The following signs and symptoms of lung infection should alert you to contact your doctor right away:

Fever

Normal body temperature varies from person to person, but in general, it’s around 98.6 F degrees. Having a fever meaning a temperature at or higher than 100.4 F is one sign that you may have a lung infection. With a fever, a person may also experience chills or shaking. 

Increased Shortness of Breath

Shortness of breath, or dyspnea, is one of the hallmark symptoms of COPD. However, if it begins to worsen, this could be a sign that you are developing a lung infection and need to contact your doctor. 

In addition to a feeling of breathlessness, rapid breathing (called tachypnea) and a rapid heart rate (called tachycardia) may also be signs of a lung infection. 

Productive Cough

All about the common cold

The common cold is a viral infectious disease that infects the upper respiratory system. It is also known as acute viral rhinopharyngitis and acute coryza. It is the most common infectious disease in humans and is mainly caused by coronaviruses or rhinoviruses.

Because there are more than 200 viruses that cause the common cold, the human body can never build up resistance to all of them. This is why colds are so common and often return. According to the CDC (Centers for Disease Control and Prevention), adults get 2-3 colds per year and children may have up to 12 per year.

The common cold is contagious; it can be spread by air droplets from coughs and sneezes and by touching infected surfaces. It is contagious from 1–2 days before symptoms begin until the symptoms have stopped.

Causes

  • When sneezing or coughing, make sure it is done into a tissue. Discard the tissue carefully and wash your hands.
  • If you sneeze into your hands, make sure you wash them with soap and water immediately.
  • If you have no tissues or a handkerchief, cough into the inside (crook) of your elbow rather than your hands.
  • Wash your hands regularly; cold viruses can be transmitted from one person to another by touch. In fact, more germs are passed by shaking hands than by kissing.

Treatments

It is important to realize that both antibiotics and antiviral medications are ineffective against most viruses that cause the common cold. A cold normally lasts up to 10 days; however, some symptoms can stay as long as 3 weeks.

Although there is no real way of treating or curing a common cold, the following measures may help ease the symptoms:

  • Drink plenty of fluids and keep well hydrated, being dehydrated when infected with a cold can make symptoms worse.
  • Get plenty of bed rest; it is important to get as much sleep/rest as possible while the immune system is fighting off the virus.
  • Take aspirin, acetaminophen, or ibuprofen to relieve headache or fever. Do not give aspirin to children under 16.
  • Some people find that inhaling steam helps ease the symptoms of nasal congestion.

What Are Peptic Ulcers?

There is no clear evidence to suggest that the stress of modern life or a steady diet of fast food causes ulcers in the stomach and small intestine, but they are nonetheless common in our society: About one out of every 10 Americans will suffer from the burning, gnawing abdominal pain of a peptic (or gastric) ulcer at some point in life.

Peptic ulcers are holes or breaks in the protective lining of the duodenum (the upper part of the small intestine) or the stomach — areas that come into contact with stomach acids and enzymes. Duodenal ulcers are more common than stomach ulcers. Comparatively rare are esophageal ulcers, which form in the esophagus — or swallowing tube — and are often a result of exposure to medications, like certain antibiotics or anti-inflammatories, or alcohol abuse.

While excessive stomach acid secretion certainly plays a role in the development of ulcers, a relatively recent theory holds that bacterial infection is the primary cause of peptic ulcers. Indeed, research conducted since the mid-1980s has shown that the bacterium Helicobacter pylori (H. pylori) is present in more than 90% of duodenal ulcers and about 80% of stomach ulcers. However, more recent figures indicate those percentages are declining.  

Other factors also seem to contribute to ulcer formation. Overuse of over-the-counter painkillers (such as aspirin, ibuprofen, and naproxen), heavy alcohol use, psychological stress, and smoking exacerbate and may promote the development of ulcers, especially in someone with H. pylori

Other studies show that stomach ulcers are more likely to develop in older people. This may be because arthritis is prevalent in the elderly, and alleviating arthritis pain can mean taking daily doses of aspirin or ibuprofen. Another contributing factor may be that with advancing age the pylorus (the valve between the stomach and duodoneum) relaxes and allows excess bile (a compound produced in the liver to aid in digestion) to seep up into the stomach and erode the stomach lining.

Also, for no known reason, people with type A blood are more likely to develop cancerous stomach ulcers.

Duodenal ulcers tend to appear in people with type O blood, possibly because they do not produce the substance on the surface of blood cells that may protect the lining of the duodenum.

Fortunately, peptic ulcers are relatively easy to treat; in many cases they are cured with antibiotics, antacids, and other drugs that reduce the amount of acid produced by the stomach. There are also a variety of self-help and alternative treatments that can aid in relieving pain. Still, the dangers associated with peptic ulcers — such as anemia, profuse bleeding, and stomach cancer– are serious, so ulcers should always be monitored by your doctor.

Scientific analysis shows that ginger contains hundreds of compounds and metabolites, some of which may contribute to health and healing. Of these, the gingerols and shogaols have been most extensively researched.

1. Digestion

To make ginger tea at home, slice 20 to 40 grams (g) of fresh ginger and steep it in a cup of hot water. Adding a slice of lemon or a drop of honey adds flavor and additional benefits, including vitamin C and antibacterial properties.

This makes a soothing natural remedy for a cold or flu.

4. Pain reduction

A study involving 74 volunteers carried out at the University of Georgia found that daily ginger supplementation reduced exercise-induced muscle pain by 25 percent.

Ginger has also been found to reduce the symptoms of dysmenorrhea, the severe pain that some women experience during a menstrual cycle.

5. Inflammation

Ginger has been used for centuries to reduce inflammation and treat inflammatory conditions.

A study published in Cancer Prevention Research journal reported that ginger supplements, which are available to buy online, reduced the risk of colorectal cancer developing in the bowel of 20 volunteers.

Ginger has also been found to be “modestly efficacious and reasonably safe” for treating inflammation associated with osteoarthritis.

6. Cardiovascular health

Other possible uses include reducing cholesterol, lowering the risk of blood clotting, and helping to maintain healthy blood sugar levels. More research is needed, but if proven, ginger could become part of a treatment for heart disease and diabetes.

What are kidney stones?


Kidney stones, or renal calculi, are solid masses made of crystals. Kidney stones usually originate in your kidneys. However, they can develop anywhere along your urinary tract, which consists of these parts:

  • kidneys
  • ureters
  • bladder
  • urethra

Kidney stones are one of the most painful medical conditions. The causes of kidney stones vary according to the type of stone.

Types of kidney stones

Not all kidney stones are made up of the same crystals. The different types of kidney stones include:

Calcium

Calcium stones are the most common. They’re often made of calcium oxalate (though they can consist of calcium phosphate or maleate). Eating fewer oxalate-rich foods can reduce your risk of developing this type of stone. High-oxalate foods include:

  • potato chips
  • peanuts
  • chocolate
  • beets
  • spinach

However, even though some kidney stones are made of calcium, getting enough calcium in your diet can prevent stones from forming.

Uric acid

This type of kidney stone is more common in men than in women. They can occur in people with gout or those going through chemotherapy. This type of stone develops when urine is too acidic. A diet rich in purines can increase urine’s acidic level. Purine is a colorless substance in animal proteins, such as fish, shellfish, and meats.

Struvite

This type of stone is found mostly in women with urinary tract infections (UTIs). These stones can be large and cause urinary obstruction. They result from a kidney infection. Treating an underlying infection can prevent the development of struvite stones.

Cystine

Cystine stones are rare. They occur in both men and women who have the genetic disorder cystinuria. With this type of stone, cystine — an acid that occurs naturally in the body — leaks from the kidneys into the urine.

Risk factors for kidney stones

The greatest risk factor for kidney stones is making less than one liter of urine per day. This is why kidney stones are common in premature infants who have kidney problems. However, kidney stones are most likely to occur in people between the ages of 20 and 50.

Different factors can increase your risk of developing a stone. Typically, Caucasians are more likely to have kidney stones than those of African descent.

Sex also plays a role. More men than women develop kidney stones, according to the National Institute of Diabetes and Digestive and Kidney Disease (NIDDK).

A history of kidney stones can increase your risk. So does a family history of kidney stones.

Other risk factors include:

  • Dehydration
  • Obesity
  • a diet with high levels of protein, salt, or glucose
  • hyperparathyroid condition
  • gastric bypass surgery
  • inflammatory bowel diseases that increase calcium absorption
  • taking medications such as diuretics, antiseizure drugs, and calcium-based antacids

Recognizing the symptoms and signs of a kidney stone

Kidney stones are known to cause severe pain. Symptoms of kidney stones may not occur until the stone begins to move down the ureters. This severe pain is called renal colic. You may have pain on one side of your back or abdomen.

In men, pain may radiate to the groin area. The pain of renal colic comes and goes, but can be intense. People with renal colic tend to be restless.

Other symptoms of kidney stones can include:

  • blood in the urine(red, pink, or brown urine)
  • vomiting
  • nausea
  • discolored or foul-smelling urine
  • chills
  • fever
  • frequent need to urinate
  • urinating small amounts of urine

In the case of a small kidney stone, you may not have any pain or symptoms as the stone passes through your urinary tract.

Why kidney stones can be a problem

Stones don’t always stay in the kidney. Sometimes they pass from the kidney into the ureters. Ureters are small and delicate, and the stones may be too large to pass smoothly down the ureter to the bladder. Passage of stones down the ureter can cause spasms and irritation of the ureters as they pass. This causes blood to appear in the urine.

Sometimes stones block the flow of urine. This is called a urinary obstruction. Urinary obstructions can lead to kidney infection and kidney damage.

Testing for and diagnosing kidney stones

Diagnosis of kidney stones requires a complete health history assessment and a physical exam. Other tests include:

  • blood tests for calcium, phosphorus, uric acid, and electrolytes
  • blood urea nitrogen (BUN)and creatinine to assess kidney functioning
  • Urinalysis to check for crystals, bacteria, blood, and white cells
  • examination of passed stones to determine their type

The following tests can rule out obstruction:

  • abdominal X-rays
  • intravenous pyelogram (IVP)
  • retrograde pyelogram
  • ultrasound of the kidney (the preferred study)
  • MRI scanof the abdomen and kidneys
  • abdominal CT scan

The contrast dye used in the CT scan and the IVP can affect kidney function. However, in people with normal kidney function, this isn’t a concern.

There are some medications that can increase the potential for kidney damage in conjunction with the dye. Make sure your radiologist knows about any medications you’re taking.

How kidney stones are treated

Treatment is tailored according to the type of stone. Urine can be strained and stones collected for evaluation.

Drinking six to eight glasses of water a day increases urine flow. People who are dehydrated or have severe nausea and vomiting may need intravenous fluids.

Other treatment options include:

Medication

Pain relief may require narcotic medications. The presence of infection requires treatment with antibiotics. Other medications include:

  • Allopurinol (Zyloprim) for uric acid stones
  • diuretics
  • sodium bicarbonate or sodium citrate
  • phosphorus solutions
  • ibuprofen (Advil)
  • acetaminophen (Tylenol)
  • naproxen sodium (Aleve)

Lithotripsy

Extracorporeal shock wave lithotripsy uses sound waves to break up large stones so they can more easily pass down the ureters into your bladder. This procedure can be uncomfortable and may require light anesthesia. It can cause bruising on the abdomen and back and bleeding around the kidney and nearby organs.

Tunnel surgery (percutaneous nephrolithotomy)

Stones are removed through a small incision in your back. This procedure and may be needed when:

  • the stone causes obstruction and infection or is damaging the kidneys
  • the stone has grown too large to pass
  • pain can’t be controlled

Ureteroscopy

When a stone is stuck in the ureter or bladder, your doctor may use an instrument called a ureteroscope to remove it. A small wire with a camera attached is inserted into the urethra and passed into the bladder. The doctor then uses a small cage to snag the stone and remove it. The stone is then sent to the laboratory for analysis.

Kidney stone prevention

Proper hydration is a key preventive measure. The Mayo Clinic recommends drinking enough water to pass about 2.6 quarts of urine each day. Increasing the amount of urine you pass helps flush the kidneys.

You can substitute ginger ale, lemon-lime soda, and fruit juice for water to help you increase your fluid intake. If the stones are related to low citrate levels, citrate juices could help prevent the formation of stones.

Eating oxalate-rich foods in moderation and reducing your intake of salt and animal proteins can also lower your risk of kidney stones.

Your doctor may prescribe medications to help prevent the formation of calcium and uric acid stones. If you’ve had a kidney stone or you’re at risk for a kidney stone, speak with your doctor and discuss the best methods of prevention.
